Online booking: Disability and Dyslexia Service - Diagnostic Funding advice reminder
Important information
A number of students are attending appointments without the required documents. This results in the appointment being cancelled and the student having to re-book. Please only make an appointment with a Disability Adviser if you are able to bring the relevant documents to the meeting.
Document Requirement Reminder
You will need:
Statements for all bank and savings accounts for both you and your partner/spouse (if applicable) within the following given time frames:
- If applying in the Autumn term: Statements for August 2012 and September 2012. i.e. showing transactions from 1st Aug to 24th Sept 2012 inclusive. This means you cannot have an appointment until at least the 24th September 2012.
- You must denote any transfers into/out of these accounts if they are over £500 and explain what they are.
Information about any children and the benefits you receive must also be identified and bank statements or entitlement letters provided.
